Where would we use a proportional font?

IIRC, icewm uses both proportional fonts.
Sylpheed also uses proportional fonts for some things.

Is proportional the two lut fonts?
What application needs the helv (monospaced?) fonts?

Would links or e3pi work with one in X?

I doubt it. Text-mode applications are usually
designed for fixed fonts.

rxvt -fn 10x20 in BL2 loads a fixed font. rxvt will not load the non-fixed fonts. Assuming BL3 is the same, you would need to add other fixed fonts to get larger/smaller or more/less text on the screen.

If BL3 has only one fixed font, and that has no extended ascii, one would need to add (or replace it with) a second fixed font that does have extended ascii.

clr9x15.pcf.gz is 2713 bytes
9x15.pcf.gz is 4591 bytes

See http://keesan.freeshell.org/bl/9x15.pcf.gz
Edit fonts.dir to replace clr9x15.pcf.gz with 9x15.pcf.gz.
(Or rename the latter to the former but that could get confusing later.)

Since BL3 has only one fixed font, could future versions come with one twice as large that does extended ascii?

The icewm and sylpheed packages could include any needed fonts.

Is it worth asking about screen size as well
as resolution in Xsetup and automatically
assigning the proper font

No, that's something users can decide for themselves.
Some people prefer a big font that's easy to see;
others prefer a small font that enables them to have
more characters on the screen.

Editing .Xdefaults to change the font is not difficult.
Nor is editing the jwm menu to change the font for a
particular item.

Cheers,
Steven
