Will it then be able to install extra programs to the image file ?

Yes. After qemu boots the image file, it treats it like a normal
drive. You can save, delete, format, whatever. You can even use
fdisk to put partitions on the image file.

Does this work similarly to loop BL3, where changes can be made as if it were running in its own ext2 partition, and they are all saved? (But I don't think loop linux lets you make partitions). The image file is mounted by qemu instead of loop?
