Yes, new Japanese beetles can come in from anywhere, but if you have a population that's been feeding on your plants and laying eggs in your grass, the adults will emerge to discover the same food their mothers had--your garden. Even though you'll be getting some immigrants every year, it really does help to kill the resident populations. I'm not sure there is anything you can do to discourage fly-ins--traps just lure them, as everyone is pointing out. But I do know that I had better luck with nematodes than milky spore in Zone 5.
As for application time: I wait until the soil has warmed up--a bit after the frost-free date. You want the grubs to have migrated back to the upper levels of the soil before you apply the nematodes. Japanese beetle grubs can feed underground for a couple of years, so I assume that even if the nematodes aren't bothering those that are about to pupate, they are attacking the younger ones.
I've had a good response to this treatment. But, like all else in the "science" of growing--there are sure to be variations from one garden to another.
Neem is another control that is said to work well on the grubs--water it in. I know that some turf people swear by aerating the sod, too--they figure the spikes kill a certain number of grubs and go over the area repeatedly. (Ugh)

Since the beetle populations have seemed to vary with the harshness of our
