It appears from your results that cs89x0 may not be
the correct module for your card. Sindi provided
that information, but I have been unable to confirm
it.

The EtherStreamer is a very old ethernet device.
You may be able to identify the module it needs
by booting an old net.i kernel from Slackware.
It will automatically try to install any network
card it finds. Here's a net.i that's about the
right age:

You can put the kernel on a DOS boot floppy with
loadlin.exe. Or if you already have a DOS partition
on the HD, you can boot it from there (using loadlin).
When the kernel boot stops, carefully read what's on
the screen. Hopefully there will be an eth0 line
that says which driver it installed for the network
card.
