Tom,

You get a plastic bottle, fill it with tap water, put the cap on and
than put a thin coat of tanglefoot (http://www.tanglefoot.com) on the
middle 2/3rds of the bottle (leave the top stick free so you can carry
them and the bottom so it doesn't attract too much dirt.). as has been
mentioned the FB's prefer white but I usually use clear bottles which
will be black with FB bodies within 4 to 5 days.

I use the handle of a plastic fork or spoon to apply tangle foot, a
putty knife would also work. Now, I have a gallon of this stuff. The
smaller containers usually have a applicator included. but if you get
some with no applicator, what ever you use should be dedicated to sticky
trap use.

If anyone uses the red sticky ball traps for fruit trees, an easy way to
to get them sticky is to coat a plastic baggie and slip that over the
red ball trap. When it is full of insect bodies just take the bag off
and toss in the garbage. If you coat the ball itself with tanglefoot you
must use either turpentine or mineral spirits to clean off the
tanglefoot (which takes about 10 to 15 minutes per ball). I figure that
plastic baggie is easier on the environment than using such chemicals
and it takes about 1 minute to coat a baggie vs about 3 minute to coat
the ball and it takes less time to remove the baggie.

Used surround last year to combat the cuke beetles and found it was not
the best choice for squash. you have to reapply the surround as soon as
there is new growth on the squash-in a good year that could be almost
daily. i also found by July the cuke beetle were eating right through
the surround.

Another possible problem is that the kaolin may be absorbing any
drifting pesticides thus either contaminating your plants with
insecticides/fungincides you don't want on your plants or worse
absorbing herbicides that could damage or kill your crop. I don't have
any science on this phenom but have been reading about this on the
garden web quite a bit lately.

Did use the stuff on apple trees as well as vegetables last year and
found it did not keep many bugs off and found plenty of fruit damage on
white (sprayed) fruit and leaves. Granted we had a wet spring so we
could not effectively apply it after petal fall so perhaps if we had had
better conditions the Surround would have worked better.
