No, you don't, and can't.

I know Frank mentioned that in the beginning but I see that "try domain login" is depreciated and that I should just use the format DOMAIN/username instead... but I don't see where a username is necessary at all according to the page listed above...

You just pass the login information to the various _connect() php function calls.

Why is freetds.conf necessary? Why can't it simply rely on PHP to make the connection to the DB as one already has to?

It's needed because there's no easy way to specify TDS protocol version, client charset and various other options elsewhere. If you can go with the built-in defaults, you don't necessarily have to use freetds.conf, but it's highly recommended.
