I have published some sheet music at the www.mutopiaproject.org under

"Creative Commons Attribution 2.5"

An example of those tunes is:

"Traditioner af Swenska Folk-Dansar, 1st part, nr 31"
http://www.mutopiaproject.org/ftp/Traditional/traditioner_af_swenska_folk_dansar.1.31/traditioner_af_swenska_folk_dansar.1.31-a4.pdf

Last week I was contacted by someone who wanted to create derivative
work ( also sheet music ) of this music. So the license requires him
to attribute my work in his derivative work.

I have read
http://wiki.creativecommons.org/FAQ#How_do_I_properly_attribute_a_Creative_Commons_licensed_work.3F

but I would be very interested in seeing some examples of how such an
attribution text could look like.

Could I require that the attribution text should be placed under each
of the tunes he creates or could I only require that the attribution
text is placed at one place in the book if he publish a collection of
these tunes?

cheers,
Erik Sjölund
