I agree that permission is not generally required to loan or even sell your copy of a book to a friend.  I think you're looking for a bright line, however there isn't one and we've never had one in any of the license versions. 

Under the 4.0 ND licenses, if you've made an adaptation of an ND licensed work, then you would only run afoul of the ND license if you distribute or communicate the adaptation in a manner otherwise exclusively reserved to original author.

So if there's no permission needed to distribute or communicate etc, then then the license doesn't come into play at all and you're free to engage in that activity (at least as a matter of copyright and similar rights).

Well, it's not quite clear how that interacts with 17 USC 109 et. al.

Permission is generally *not* required to loan a copy of a book to a friend. Can I lend a copy of an adapted version of an ND work to a friend? What if I charge the friend? What if they're not friends, and I made 10,000 copies?
