1. Concerning Mishnaic Hebrew, I don't know what was there in the background and what was "developed".

2. מי הוא זה ואי זה הוא אשר מלאו לבו to say adan for adam? Did he mean ADON?

3. Concerning YITFOS יִתְפֹּס, it is not god given that the BKP letters must be "hardened" upon the sight of a dot inside them. The common Hebrew speaker evens out matters by comparing TAFAS to YITFOS. He does it also with MASAX[K], 'screen', MASAX[K]IYM, 'screens'. He, nevertheless, says YIZKOR with a hard K, and also +IPES, 'climbed', with a hard P.
