answering myself. i think i filed for the office visit, but it wasn't
covered. after checking on some websites, i see only if you're getting
tested and fitted for a hearing aid (not ear protection) does insurance
(mine's the state health plan for state employees) cover the doctor
visit. it's been a couple of years and i'm having trouble remembering.
