I wouldn't read that as saying you must note the
changes that you've made. I would read that as
saying you must acknowledge that you used
someone else's work in your derivative work.

Something like:
"cover based on photo by Alice Smith"

Not something like:
"cover based on photo by Alice Smith with the
bison cropped out and replaced with a flamingo"
