A few other things to check (before inserting card):

1) Make sure you have the correct modules compiled:
modprobe -l | egrep "(pcmcia_core|i82365|ds)"

2) Check and make sure your network options are configured in
/etc/pcmcia/network.opts

3) Verify that cardmgr is running:
ps ax | grep cardmgr
