In Australia, for any competitive equestrian sport, we don't have the
option - helmets (rated to the appropriate standards) are mandatory.
Our MSAR unit being created will have helmets as mandatory equipment. We
cannot get insurance without them.
I used to ride without, but since switching (a few years ago) I'd never
go back. We have clip-on all-round visors for sun protection, or most
helmets come with a front peak visor if that's all you need.
Having said that, at least one manufacturer is looking at an
Akubra-style hat-helmet but the work to create one that meets the
standards AND still looks cowboy-ish is not going smoothly.
