I'd like to share the form but do not know of a way to do that without giving everyone "access" to the associated spreadsheet.  If you send people the form, they can automatically use it and then whatever they input into the form goes into my spreadsheet.  The form and spreadsheet are linked.  (When you create a "form" you are really creating a spreadsheet...the form is just a means of populating the cells of the spreadsheet.)

Speaking of which, Google Doc Forms would be an excellent way to administer a CSA member survey or any other situation where you want people to provide you with information.  Responses are automatically recorded in the linked spreadsheet.  You can imbed a Google Doc form into an email message or onto a web site.

Using "Forms" is really quite simple.  Just go to Google Docs and click on "forms" and get started.

John – would you share your Google Docs form. I have not used Google docs yet, but am interested.

I have found Quickbooks easy to use and many in my area use it as well.  I supplement it with spreadsheets for planning and other types of recordkeeping and reporting.  One thing I am doing this year is utilizing a Google docs spreadsheet "form" so that I and my one employee can track labor hours by crop.  Not sure how I'll account for my two sons' time (their hours are erratic and not always very productive!).  We will be completing the "form" daily and it automatically populates a spreadsheet.  We will then use a spreadsheet tool from the University of Wisconsin called Veggie Compass to analyze profitability of each crop.  In the interest of full disclosure, I am part of a team at UW working to build and hone the Veggie Compass spreadsheet tool.  It is a free.  Download the current version here:

Veggie Compass

As improvements and changes are made to VeggieCompass, newer versions will be posted.  Our team welcomes input and feedback.
