I had to do this the very first day the greenhouse was completed and (for some crazy reason) our puppy attacked the poly on one side chewing a huge hole in it.  Sigh. 
 
The patch held for several years.  Here's what I did.  Clean and dry the poly so that the repair tape has a surface free of dust (or puppy slobber).  I cut 2 patch pieces of left over poly to cover the hole and extend beyond the raw edge 3 inches.  Tape one piece of poly to the outside of the greenhouse and tape the other piece to the inside of the greenhouse.  This may be excessive but I was more comfortable knowing that if the outside piece somehow came off there was still the inside piece to back it up.  Taping the patch piece just to the inside won't work because rainwater collects between the patch and poly and causes the tape to fail.
