The getting_started doc was really intended towards developers, not users.
I guess a README in the doc directory should state what each file is.

Actually the first sentence says that pretty plainly:

"This document is intended for people who want to help on the FreeTDS
project and/or are interested in how the code works."

Not sure were the confusion comes in.

Also, I think I would added a second note (**) next to MS SQL with
something like this:

** It is possible under some versions of MS SQL to send a TDS 5.0 login
packet which will cause the server to negotiate a 4.2 connection. Support
for this is spotty at best and should be avoided.
