If I may contribute some information to the discussion I should say that the Greco-Roman East following each own local traditions deified and worshipped the emperors while still alive. For example the people of Thessalonica worshipped Augustus and Claudius attributing to them the title "God". Living emperors could also do miracles, vgl. the case of Vespasian in Egypt if I am not wrong.

I think that depends upon the Emperor.  If memory serves, both Caligula and Nero had themselves deified while alive,  but "the Divine Julius" and "the Deified Augustus" were done posthumously.
