From the 30th Chaos Communication Congress just emerged a very handy tool to collect those examples that are asked for in the EU questionnaire, in a very easy way: http://okfde.github.io/eucopyright/en/30c3/
Please spread the word about that page. We'll all get access to the heap of answers that will hopefully come in.

that would certainly be helpful. with regards to the territoriality question i would urge anyone interested in dealing with these questions to take a look at the third proposal Hugenholtz and Hargreaves make in this paper (on page 10-11): http://www.lisboncouncil.net//index.php?option=com_downloads&id=847

communia recommendations 4, 6 and 7 come to mind. generally the communia policy recommendations (http://www.communia-association.org/recommendations-2/) probably provide a good starting point for answering most of this. see also here: http://www.communia-association.org/2013/12/11/commission-announces-public-consultation-on-the-review-of-eu-copyright-rules/
