It appears that the /dev/root designation is coming from mount.
I got the same thing in BasicLinux when I used the busybox
version of mount. The /dev/root disappeared when I switched
to the standard mount (from Slackware 7.1).

> user@7[/]$ mount -V
> mount: mount-2.11y

The mount-2.11 looks like a standard version; although, I've
never seen that y before. I suppose it means the 25th revision
of 2.11 ?

Another approach to finding the source of this is to look at
the partition and the parameters used to mount that partition.
Was it formatted (mke2fs) by the same system that mounts it?
Were any unusual parameters used to format and/or mount it?
For example, I have never seen that errors=remount-ro before.
I've just looked it up (man mount) and it appears to be legal,
but how exactly did it get there?
