I am a backyard gardener, so I buy a little of this and a little of that. As such, you should beware that I haven't bought enough of anything form any source to have rigorously tested it. I might have just been lucky or unlucky. That being said, I have bought northern highbush blueberries from the following sources east of the rockies:

Nourse: I like this company because they are one of the few that rates their stock comparatively and will say "this one doesn't taste as good as the others". Everything I've bought from them has arrived in good condition and grown well except for some raspberries I got this year. (I sent them a note describing my problems, and they were very aggressive in contacting me to arrange a replacement.) The blueberries and rhubarb that were packed with the raspberries are looking very good, and the currants, blueberries, and strawberries I've bought from them in the past have all been good.

Hartmanns: They have an enormous selection. I've had more deaths from their stock than from some others, but I suspect the problem was the spot I chose and lack of adequate water.

St. Lawrence Nurseries: They mostly have stuff for further north. I bought half-high blueberries from them. (A cross between the northern highbush and the eastern lowbush blueberry.) Maybe not what you are looking for, but this company seems to be a small operation that grows more than it sells. If you buy something they have had for several years you often get oversized plants. I've had very good luck with plants from St. Lawrence, and generally choose them as my vendor if several places carry the plant I want.
