Really briefly because I should be outside putting broccoli in the ground, but

to be a CSA
farm, do I need to join some sort of government group

no. There isn't such a thing

or have some sort of
certificate or license to be a CSA farm?

no

Is there a master list of
"official" CSA farms,

no

or are they CSA farms just because they say they are
and post their name on a web site?

there are two nation lists you can register with (and a number of other local ones

We already have a Farmers' Market group
that has become a nonprofit organization, of which I'm on the board Can
that group be a CSA group somehow, being the focal point for farmers and
businesses to get together, or does it have to be by individual farms
working independently?

Actually, both models are out there. Look at the webpage that Allan recommended (Robyn Van En CSA Center )for a good place to start. I think a google search of CSA might give you more of an idea. Are there books out there? probably, but I haven't seen anything that I would recommend - but that doesn't mean there aren't any.
