No, nothing at all needs config.h unless HAVE_CONFIG_H is defined. That's
the whole point. You can use some other mechanism to pass the needed
#defines. In fact, if you remove the line
AC_CONFIG_HEADER(include/config.h)
from configure.in, and rerun your autogen, you will see that FreeTDS will
still build properly. It will pass all the required flags on the command
line. (You may need to cvs update first; I just fixed tsql.c and freebcp.c
a few minutes ago).
One could provide a Makefile.win32 (or whatever incompatible stupidity the
Microsoft compiler might use instead of Makefiles) that passes all the
required defines to the
