Joshua, this is an interesting approach, my comments below

All Creative Commons licenses include the right to distribute and broadcast http://creativecommons.org/learn/licenses/fullrights), so this question may need to be rephrased or eliminated.

As it stands, if someone clicked "no", they should not get a CC license. A "yes, however they wish" sounds like it would map to a Public Domain license. The others are nice obvious modifiers, but I don't understand the "not alongside any other media" option. What license property does that map to?

The rest of the options are logically arranged, getting more restrictive as you go down the list. I'm assuming you were envisioning these options as checkboxes (so you could click as many as necessary), correct?

Technically, it doesn't seem impossible to map these questions to the current set of licenses, though mixing new license properties into old ones would throw a wrench into the current backend.
