OK, I'm exhausted from all those. Sorry for such a long list, but it's
a really complicated subject. There are probably a lot more edge cases
that need exploration.

This is a great list and exactly the sort of thing CC should have as part of their FAQ.

Howabout:

* An individual downloads a by-nc 2.0 book on their computer and
pays their local copy shop to print and bind them a copy.
They then sell this on eBay.

* An individual downloads a by-nc 2.0 book on their computer and
pays their local copy shop to print and bind them a copy.
They then give this to a friend, who sells this on eBay.

i.e. how does NC interact with "first sale".
