This is just a matter of getting your freetds.conf right. You should test
it with tsql -- it works on Win32, yes? -- before you try PHP.

Just put your dataserver's IP address (4 numbers, no port) on your "host"
line. I think that's all you'll need to get yourself connected.

(I knew it was a Windows box by the logfile name and the fact that
connect(2) showed a failure in the log without a valid error code.)

What you have here is pilot error. But you can blame Windows if you like.
