Sam,
"Pinch a queen" is short for "pinch her head off", which is exactly what it says. It is the generally accepted way for a beekeeper to "retire" a queen, aka "give her a gold watch."
A bee's life isn't all that great.

What does pinch a queen mean? Exterminate her?

I would think the decision in some part would depend on whether you were
thinking about replacing her with a mated queen or virgin queen.
If a mated queen, if there is uncapped brood in the hive you should be fine
pinching her now. I mated queen should be laying eggs within 10 days or so
of introduction in the hive.

I have one queen who is very naughty. She has a very poor brood pattern,
little or no honey and very weak pollen in the hive. This is a brand new
wooden hive with brand new foundation and brand new bees. The queen went in
on Feb 2nd.
If I should pinch, when is the best time? I recall Randall saying to pinch early, but
there must be a "best" time of day/brood Cycle.
Or maybe I could add some drawn out wax to give her some quick room to get
started with a bunch of eggs so the the brood is built up and then pinch her?
Thoughts?
