My nieghbor has onion plants shipped in from the south and for years spent many hours replanting as he would find them bottom side up in bunches around his onion patch each morning.Finally he took a light, a chair, and six pack and went out there after dark and discovered that night crawlers dote on the dead and dieing outer skins of the plants and try to drag them underground, having failed with one they get another resulting in the bunches or 4 to 6 plants.
The cure was simple, peel of any scruffy outer layers and plant only the clean healthy plants.
