The best example I know about is Rolling Prairie Farmers Alliance of Lawrence Kansas.  Are you acquainted with "Growing for Market" magazine?  The editor's husband is a member of the alliance; in fact I think he may have started it.
 
There are other cooperatives like Carolina Organic Growers, Inc of Marshall, NC and Appalachian Sustainable Development in SW VA, but I do not think that these two have CSAs.

Can anyone tell me about or give the url of examples of "cooperative" or "collective" CSAs, i.e., where — in order to expand the offerings — supplies are gained from a number of neighboring farms and/or tying into a buying club for items like dairy, grain, nuts, etc. that any one CSA might not provide.
