Ok, It's definitely three phase. It can be run from 400 when wired as star or 220 when wired as delta. If you're lucky, when you take the lid of the motor's connection box, you'll find six connections and three links, something like this:-
L1, L2 & L3 are the connections into the motor. Wired as shown, it'll be 220V. If the links go between W2, U2 & U2, then it'll be 400V.
