The web site I came across for mycorrhizal quality
evaluation was part of Reforestation Technologies
International in California.
RTI Internal Standards for Quality Assurance of
Mycorrhizal Inoculum

They describe their internal standards for quality
assurance. But they also mention a fee-based
service for evaluation of mycorrhizal colonization
on roots.
RTI suggests BBC Labs in Arizona as a
commercial lab for mycorrhizal analysis,
as well.

INVAM - International Culture Collection of (Vesicular)
Arbuscular Mycorrhizal Fungi - has a series of
leaflets and papers that describe culture methods
for mycorrhizal inoculum, spores, and fungal species.
