I think it is a tall order to incorporate all those things, but...
You might look at the 2 volumes by Alister McGrath. One volume is his
theology but the other volume is a book of readings on the traditional areas
of theology. He begins with the 2nd century and goes through the present,
giving a fairly broad representation of views. It is this second volume
that helps give a historical survey.
