To me the more important questions are:
1. Is the including of a piece of music in a movie a derivation of the original work even if the work is left untouched?
the 2.0 versions of the licenses clarify this point -- "synching", ie using the music as soundtrack material for film or video, is to be considered for the purposes of the license as creating a derivative work -- you can read this clearly in the text of the share-alike licenses:
"<x-tad-smaller> For the avoidance of doubt, where the Work is a musical composition or sound recording, the synchronization of the Work in timed-relation with a moving image ("synching") will be considered a Derivative Work for the purpose of this License."
