Creative Commons doesn't assert copyright over the text of the
licenses. So, you can copy the by-nc-sa license as a template, and add
whatever extra stuff you want on there.
The only catch is that "Creative Commons" is a trademark, so you can't
call your final product a "Creative Commons" license.
