GKC (28e, page 93) attributes this to the Masoretes, and "they do not
alter the monosyllabic character of the forms" (94).

> And when did (Proto)Hebrew drop case endings? And how do we know it?

Waltke/O'Connor claim, "During the last centuries of the second
millennium, the case system disappeared." They say we know it from
Akkadian, Classical Arabic, Early Northwest Semitic, Ugaritic, and
Canaanite glosses in the Armana letters (page 126).
