I have not yet seen macros which convert selected portions of files, but SIL has developed whole file conversion CCTs for the MCW scheme which result in the correct codes for their SIL Ezra font. (It's a three-step process, but very do-able.) You can find them at http://www.sil.org/computing/sil_computing.html. Download and install the SIL Hebrew Font System, and you'll find the tables under CCTs. You'll also find the conversion process described in TechDocs.pdf, which is found under Docs.
Because "`" is an optional character in our scheme, you might want to change the bhs2de.cct to include these conversions. (The SIL scheme already uses "'" for aleph, so the need for a change for this optional character is negated.) Simply edit the file on any text editor and find the line:
"(" > "v" c ayin
Insert a line after it and type:
"`" > "v" c ayin
Make sure you save the file as a text file, and it should convert both options with no problems.
Of course, to use these conversion tables, you'll need SIL's cc.exe or ccw.exe, but these can be downloaded from SIL's ftp site, which is linked to the above site as SIL Software. You get a copy of cc.exe with CC75UG (the user's guide files for the DOS version), which is found under DOS files. Or you could get a copy of the Windows version, ccw.exe, through pulling in the SFC files, found under Windows files. Personally, I find the Windows version to be much more convenient, as it keeps a command history and can be modified easily.
