I think I got mine here.: http://www.gardeninsects.com/index.asp   (It's been at least 3 years, maybe 4) I ordered a collection of beneficial insects that included, green lacewings, lady bugs, nematodes and praying mantis.  But they ran out of praying mantises and sent me double lady bugs, instead.  They don't appear to have mantids in the collection any more.

I think his name is Phil.  My advice: call, don't email.  They're not
quite 21st century with their web infrastructure yet.  And he might
have a hearing/handwriting issue (or I have a speaking problem).  It
took 4 tries to get my delivery address correct...  But they showed
up, I crossed my finger and applied them.  I split an order with 3
other beekeepers.  None of them have terrible SHB problem.  One of the
three has higher levels, but nothing near what I saw yesterday (I'm
not convinced he applied them correctly or put them near his hive- he
might have used them in his garden instead).  It's no silver bullet,
but it's part of IPM in my opinion...
