Sorry, I should have said this before... what is the path to your
freetds.conf? On Fedora/RedHat linux it is usually /etc/freetds.conf and on
Ubuntu/Debian it is /etc/freetds/freetds.conf

Of course these paths could be based on FreeTDS versions, but I just know
what I went through recently. I got the exact same error you got and fixed
it by exporting FREETDSCONF equal to the path to the FreeTDS conf file.

Also make sure your freetds conf file is set up properly. No matter how hard
I tried to connect by host name and port i was never able to do that on both
Fedora and Ubuntu. It was only after I made an entry in the freetds.conf and
tried to connect with the servername alias that it would connect.

Here are a few links for what I have done to make it work on both Ubuntu and
Fedora:
http://www.robert-gonzalez.com/2009/02/18/building-the-php-ms-sql-server-extension-from-source-on-ubuntu-810/
http://www.robert-gonzalez.com/2008/03/31/connecting-php-on-linux-to-mssql-on-windows/

Both of those are specifically geared toward getting PHP to talk to SQL
Server using its own extension. However in order to do that you have to be
able to use tsql at the command line. In both cases I was able to get tsql
to connect.

HTH, this is not really a strong suit of mine but I know it is very
frustrating when it doesn't work like you expect it to. ;)
