More precisely, it *is* well-formed, but it is not
"namespace-well-formed", a term defined in the errata to Namespaces in
XML <http://www.w3.org/XML/xml-names-19990114-errata>. Such documents
have no infoset, and XOM chooses (wisely, IMHO) not to try to handle them.
