Just a guess, but you may be trying to add an additional namespace declaration for the default namespace to an element that's already in the default namespace. That is, you may be trying to do this:

<element xmlns="http://www.example.org/"; xmlns="http://www.somewhereelse.org/"; />

You can't do that in either XML or XOM. Or maybe you're binding a prefix twice. I can probably improve the exception message a little so it includes the prefix. maybe that will help debug this.
