disagree on the codling moth diagnosis - generally there would be frass at the calyx and one exit hole -
this appears to be a couple weeks or more old as some secondary deterioration is developing -
did you have a hail storm? - - I really don't recognize the damage,  it's not one of the big three (curculio,  apple maggot,  codling moth) if it is insect,  it would appear to be a feeding scar,  with the skin punctured to access the juice - something with piercing/chewing mouthparts like a grasshopper perhaps?
