Yes, that was the prefix location specified at configuration time. Shouldn't
be an issue though because it is looking at the right path according to the
log, right?

It sure looks like libodbcinst.so.1 wants to load
/usr/local/freetds/lib/libtdsodbc.so.0.0.
That's not the default location, although it's used by some RPMs. If you're
quite sure that "ls -l /usr/local/freetds/lib/libtdsodbc* is showing you a
complete set of executable files, then I guess I'd wonder about the version
of ld.so.1 or something like that.
For the record, here're mine:
$ ls -l /usr/local/lib/libtdsodbc* |cut -b'-11,53-'
-rw-r--r-- /usr/local/lib/libtdsodbc.a
-rwxr-xr-x /usr/local/lib/libtdsodbc.la
lrwxr-xr-x /usr/local/lib/libtdsodbc.so -> libtdsodbc.so.0.0
lrwxr-xr-x /usr/local/lib/libtdsodbc.so.0 -> libtdsodbc.so.0.0
-rwxr-xr-x /usr/local/lib/libtdsodbc.so.0.0
