This will install /usr/local/etc/freetds.conf.

Do you mean /usr/local/etc/freetds.conf or /usr/local/freetds/freetds.conf?

Normally we recommend you use the server's DNS name, but this will work.

Not needed, because you configured "--with-tdsver=7.0", but OK.

I don't have any advice for you here. Based on the text of the message, it
looks like the PHP mssql extension isn't really compiled in.
