Chris,
That '40 seconds' smells like a DNS lookup timeout.
Check that the database server can resolve the IP address it sees the
connection coming from. 'nslookup n.n.n.n' If the intervening firewall
uses NAT, then the IP address will match the firewall's interface
closest to the database server.
Gather more information on the connection to the database server, using
tcpdump or similar on the interface of the machine making the
connection. Examine the timings of packets to determine where the delay
is occurring.
