Something is missing from CVS (the tds.h.in file) so all I did was to
(a) recheckout tds so I had a fresh copy of tds.h, (b) save that copy of
tds.h elsewhere in case it gets munged again, (c) go into the include/
directory and touch tds.h, (d) edit the Makefile in the include/
directory to remove the stanza for building tds.h from tds.in.h, (e)
change back to the top directory and build (i.e. run make).

It's an annoying gotcha but by no means a show stopper. Perhaps tds.h.in
will get checked in soon.
