I have a copy of the CCAT parallel electronic text, so I tried looking through it (my search technique probably left something to be desired). Here's what I found (assuming I've interpreted it correctly):
Sir 11:4; 36:8 appear to have יי (YY) which the Greek renders κυριος (kurios).
Sir 11:12, 14, 21; 15:18; 16:2; 35:24; 40:26; 44:16; 45:21; 46:3, 6, 10, 19; 50:13, 20, 29; 51:12 appear to have ייי (YYY) which the Greek renders κυριος (kurios).
I would not assume this to be either correct or comprehensive, but perhaps it goes some way towards answering your query.

A question has arisen that really stirred my interest and I cannot find an answer to it so I ask you learned nobles. I've skimmed, scanned and turned pages in Beentjes edition of the Hebrew text of Ben Sira and cannot find YHWH. I don't have a concordance to the Hebrew Ben Sira- so if someone has access to the *Konkordanz zum hebraischen Sirach*, by Rickenbacker, they might be able to look up the use of the divine name in a moment.
Thanks
