I have just fought against this issue too. Seems the problem is with
/etc/udev/udev.conf file. We normal use our own udev.conf from spell directory.
Now the udev.conf from udev tarball is installed. This one does not contain
udev_root="/dev" and seems this is the problem. When I replaced
/etc/udev/udev.conf with udev.conf from spell directory seems it works like a
charm.

INSTALL:
udev.conf is installed by 
install_config_file "$SPELL_DIRECTORY/udev.conf" \
                    "$UDEV_CFG/udev.conf" &&

but it is replaced by udev.conf from udev tarball by default_install


Why only some boxes have problem? Castfs issue?


BTW: If you have this problem try:
mount -o remount,rw /
telinit run udevd restart
telinit run coldplug start
mount -a
replace udev.conf or add udev_root="/dev" to it
