[freetds] trigger failes, mssql_query returns true

I doubt I can help you directly, because you have a PHP question, not a
FreeTDS one.  (The problem is probably that FreeTDS differs somehow from
Microsoft's db-lib implementation, but we don't have enough
information.)  

If you want to try to track it down, look at (or ask on a PHP list) how
PHP uses db-lib.  I can tell you that the db-lib function dbsqlexec()
returns SUCCEED most of the time, even if a trigger fails.  A db-lib
application learns about errors via a callback to the message handler
(dbmsghandle).
