Hosanna comes form the Hebrew test of Psalm 118.25, and basically means: "Save now!" "In the highest" in Matthew, I feel, is a circumlocution for the divine name. You might want to check Luke 19.39, which does not contain the word Hosanna, but writes a little commentary on the Matt, Mark, and John passages: "Peace in heaven and glory in the highest!"
