Well, the majority of the population who have health insurance receive good care presumably, but 41 million US citizens don't have health insurance:

Would you find it acceptable that the majority of US children received the world's best education while the poorest received no education at all? That's why the US supports public schools, even though many choose to pay extra to send their children to get a better education in private schools. It provides a minimal level of care so that all have at least a shot at a better life, while not restricting those with money for paying for an extra level of services.

The same would be true of a universal health care system, providing a minimal level of care, while not preventing those with money from paying for extra services.

Note that the US already provides health care for the elderly, poor, and children. The 41 million consist of the working poor who make too much to qualify for a aid, or have a job that doesn't provides health care or doesn't pay enough to afford private insurance.
