Failure to connect is either failure to find the server or failure to
authenticate, probably the former.

One guess: you're using
"/usr/local/etc/freetds/freetds.conf".

The default location for that file is
"/usr/local/etc/freetds.conf".

Unless you changed the default at configure time, FreeTDS is not reading
your freetds.conf.

If that's not the problem, I suggest you try TDSDUMP and TDSDUMPCONFIG
to make sure your file is being read, and to see what response is coming
back from the server.
